Effect of number of uses and sterilization on the instrumented area and resistance of reciprocating instruments

Objectives

This study evaluated the effect of repeated uses and autoclaving in the instrumented area, fracture resistance, and time of instrumentation of thermally treated nickel-titanium reciprocating systems.

Materials and Methods

Two hundred simulated canals were instrumented using Reciproc Blue and WaveOne Gold. Each file was used up to 10 times or until fracture. The instrumented area was measured in pre- and post-operative images, using ImageJ software. Kaplan-Meier survival analysis evaluated the number of uses of instruments before fracture. Instrumented area and time of instrumentation were analyzed by Mann-Whitney U test and Kruskal-Wallis. Correlations among the number of uses and instrumented area were measured. The level of statistical significance was set at p < 0.05.

Results

Reciproc Blue presented a higher estimated number of uses in comparison with WaveOne Gold (p = 0.026), but autoclaving did not affect the resistance to fracture of instruments (p > 0.05). The instrumented area was different among the evaluated groups (p = 0.039), and the instrumented area along the uses of both tested instruments was reduced. With the time of instrumentation, there was also a significant difference among the evaluated groups; the groups without sterilization cycles were faster, in comparison to those submitted to autoclaving (p = 0.010).

Conclusions

Reciproc Blue was more resistant than WaveOne Gold, suffering later fracture. Additionally, the sterilization cycles did not influence the estimated number of uses of thermally treated reciprocating instruments, but the instrumented area of root canals was reduced along with the repeated uses of both instruments.

Comparison of vibration characteristics of file systems for root canal shaping according to file length

Objectives

No studies have yet assessed vibration characteristics according to endodontic file length. Accordingly, the objective of the present study was to examine the vibration characteristics according to nickel-titanium file length and to compare these characteristics between different file systems.

Materials and Methods

A total of 45 root canal models were divided into 3 experimental groups (n = 15 each) based on the file system used (ProTaper Gold [PTG], ProTaper Next, or WaveOne Gold [WOG]). Each experimental group was further divided into 3 subgroups according to file length (21, 25, or 31 mm). An electric motor (X-SMART PLUS) was used in the experiment. For each file system, vibrations generated when using a size 25 file were measured and used to calculate the average vibration acceleration. The differences in vibrations were analyzed using 1-way analysis of variance and the Scheffé post hoc test with a confidence interval of 95%.

Results

In the PTG file system, significantly lower vibration acceleration was observed when using a 21-mm file than when using a 31-mm file. In the WOG file system, significantly stronger vibration acceleration was observed when using a 31-mm file than when using 21- or 25-mm files. Regardless of the file length, the WOG group exhibited significantly stronger vibration acceleration than the other 2 experimental groups.

Conclusions

In clinical practice, choosing a file with the shortest length possible could help reduce vibrations. Additionally, consideration should be given to vibrations that could be generated when using WOG files with reciprocating motion.

Cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider, and the One G glide path instruments in double-curvature canals

Objectives

The aim of this study was to compare the cyclic fatigue resistance of the WaveOne Gold Glider, ProGlider and One G glide path instruments in artificial double-curvature canals.

Materials and Methods

This study included 15 WaveOne Gold Glider (size 15/0.08), 15 ProGlider (size 16/0.08), and 15 One G (size 16/0.06) nickel titanium files. The files were used in accordance with the manufacturer's instructions until they were broken in artificial double-curvature canals made of stainless steel. The time to fracture was recorded via a digital stopwatch and the number of rotations until fracture was also calculated. The data were statistically analyzed via the Kruskal-Wallis test.

Results

The highest average number of rotations until fracture of the files was found for the WaveOne Gold Glider, followed by ProGlider and One G in order. Statistically significant differences were present between all groups of files (p < 0.05).

Conclusions

In our study, the resistance of the WaveOne Gold Glider nickel-titanium (Ni-Ti) file to cyclic fatigue in S-shaped curved canals was found to be higher than that of the ProGlider and One G Ni-Ti files.

Objectives

The aim of this study was to investigate the effect of glide path preparation with PathFile and ProGlider nickel-titanium (NiTi) files on the cyclic fatigue resistance of WaveOne NiTi files.

Materials and Methods

Forty-four WaveOne Primary files were used and divided into four groups (n = 11). In the first group (0 WaveOne), the WaveOne Primary files served as a control group and were not used on acrylic blocks. In the 1 WaveOne Group, acrylic blocks were prepared using only WaveOne Primary files, and in the PF+WaveOne group and PG+WaveOne groups, acrylic blocks were first prepared with PathFile or ProGlider NiTi files, respectively, followed by the use of WaveOne Primary files. All the WaveOne Primary files were then subjected to cyclic fatigue testing. The number of cycles to failure was calculated and the data were statistically analyzed using one-way analysis of variance (ANOVA) and the Tukey honest significant difference multiple-comparison test at a 5% significance level.

Results

The highest number of cycles to failure was found in the control group, and the lowest numbers were found in the 1 WaveOne group and the PF+WaveOne group. Significant differences were found among the 1 WaveOne, PF+WaveOne, and control groups (p < 0.05). No statistically significant differences were found between the PG+WaveOne group and the other three groups (p > 0.05).

Conclusion

Glide path preparation with NiTi rotary files did not affect the cyclic fatigue resistance of WaveOne Primary files used on acrylic blocks.

Comparison of the shaping ability of novel thermally treated reciprocating instruments

Objectives

The present study aimed to evaluate the shaping ability of 2 thermally treated nickel-titanium reciprocating systems in simulated curved canals.

Materials and Methods

Forty simulated canals were prepared to apical size 25 using Reciproc Blue R25 (VDW) and WaveOne Gold Primary (Dentsply Sirona) instruments. Standard pre- and post-preparation images were taken and superimposed. The removal of resin material was measured at 5 standard points: the canal orifice, halfway between the canal orifice and the beginning of the curve, the beginning of the curve, the apex of the curve, and the end-point of the simulated canal. The data were analysed using the independent sample t-test with a 5% significance threshold.

Results

The canals in which Reciproc Blue R25 was used showed a significantly greater widening than those in which WaveOne Gold was used at 4 of the 5 measurement points (p < 0.05). The Reciproc Blue R25 instrument removed significantly more resin from the inner aspect of the curve at 2 of the 5 points and similar amounts at the remaining 3 points. At the 2 apical points, there was no significant difference between the Reciproc Blue R25 and WaveOne Gold Primary instruments.

Conclusion

Both instruments respected the original canal anatomy; however, WaveOne Gold resulted in a more conservative shape with less transportation.

The effect of root canal preparation on the surface roughness of WaveOne and WaveOne Gold files: atomic force microscopy study

Objectives

To examine the surface topography of intact WaveOne (WO; Dentsply Sirona Endodontics) and WaveOne Gold (WOG; Dentsply Sirona Endodontics) nickel-titanium rotary files and to evaluate the presence of alterations to the surface topography after root canal preparations of severely curved root canals in molar teeth.

Materials and Methods

Forty-eight severely curved canals of extracted molar teeth were divided into 2 groups (n = 24/each group). In group 1, the canals were prepared using WO and in group 2, the canals were prepared using WOG files. After the preparation of 3 root canals, instruments were subjected to atomic force microscopy analysis. Average roughness and root mean square values were chosen to investigate the surface features of endodontic files. The data was analyzed using one-way analysis of variance and post hoc Tamhane's tests at 5% significant level.

Results

The surface roughness values of WO and WOG files significantly changed after use in root canals (p < 0.05). The used WOG files exhibited higher surface roughness change when compared with the used WO files (p < 0.05).

Conclusions

Using WO and WOG Primary files in 3 root canals affected the surface topography of the files. After being used in root canals, the WOG files showed a higher level of surface porosity value than the WO files.

Comparison of cyclic fatigue life of nickel-titanium files: an examination using high-speed camera

Objectives

To determine the actual revolutions per minute (rpm) values and compare the cyclic fatigue life of Reciproc (RPC, VDW GmbH), WaveOne (WO, Dentsply Maillefer), and TF Adaptive (TFA, Axis/SybronEndo) nickel-titanium (NiTi) file systems using high-speed camera.

Materials and Methods

Twenty RPC R25 (25/0.08), 20 WO Primary (25/0.08), and 20 TFA ML 1 (25/0.08) files were employed in the present study. The cyclic fatigue tests were performed using a dynamic cyclic fatigue testing device, which has an artificial stainless steel canal with a 60° angle of curvature and a 5-mm radius of curvature. The files were divided into 3 groups (group 1, RPC R25 [RPC]; group 2, WO Primary [WO]; group 3, TF Adaptive ML 1 [TFA]). All the instruments were rotated until fracture during the cyclic fatigue test and slow-motion videos were captured using high-speed camera. The number of cycles to failure (NCF) was calculated. The data were analyzed statistically using one-way analysis of variance (ANOVA, p < 0.05).

Results

The slow-motion videos were indicated that rpm values of the RPC, WO, and TFA groups were 180, 210, and 425, respectively. RPC (3,464.45 ± 487.58) and WO (3,257.63 ± 556.39) groups had significantly longer cyclic fatigue life compared with TFA (1,634.46 ± 300.03) group (p < 0.05). There was no significant difference in the mean length of the fractured fragments.

Conclusions

Within the limitation of the present study, RPC and WO NiTi files showed significantly longer cyclic fatigue life than TFA NiTi file.

Objectives

To compare the fatigue life of Tango-Endo, WaveOne GOLD, and Reciproc NiTi instruments under static model via artificial canals with different angles of curvature.

Materials and Methods

Reciproc R25, WaveOne GOLD Primary, and Tango-Endo instruments were included in this study (n = 20). All the instruments were rotated in artificial canals which were made of stainless steel with an inner diameter of 1.5 mm, 45°, 60°, and 90° angles of curvatures and a radius of curvature of 5 mm until fracture occurred, and the time to fracture was recorded in seconds using a digital chronometer. The data were analyzed using Kruskal-Wallis and post-hoc Dunn tests were used for the statistical analysis of data in SPSS 21.0 software.

Results

Tango-Endo files were found to have significantly higher values than WaveOne GOLD and Reciproc files in terms of fatigue life (p < 0.05). However, there was no statistically significant difference between fatigue life of Reciproc and WaveOne GOLD files (p > 0.05). It was determined that increasing the angle of curvature of the stainless canals caused significant decreases in fatigue life of all of three files (p < 0.05).

Conclusions

Within the limitations of the present study, the cyclic fatigue life of Tango-Endo in canals having different angles of curvature was statistically higher than Reciproc and WaveOne GOLD.

Influence of operator's experience level on lifespan of the WaveOne Primary file in extracted teeth

Objectives

The aim of this study was to assess the influence of operator experience level on the lifespan of the WaveOne Primary file (Dentsply Maillefer, Ballaigues, Switzerland) in extracted teeth.

Materials and Methods

Moderately curved canals of extracted maxillary and mandibular molars were randomly distributed into 2 groups: experienced and inexperienced operators. Ten files were allocated to each group (n = 10). Each canal was prepared until the working length was reached, and the same file was used to prepare additional canals until it separated. The number of canals prepared before file separation was recorded. The fragment length of each file was measured, and the location of the fragment in the canal was determined. Data were statistically analysed using the independent 2-sample t-test.

Results

The 2 operators prepared a total of 324 moderately curved canals of maxillary and mandibular molars. There was no significant intergroup difference in the mean number of canals prepared (p = 0.27). The average lifespan of the WaveOne Primary file was 17.1 and 15.3 canals, and the longest lifespan was 25 and 20 canals, when used by experienced and inexperienced operators, respectively. There were no statistically significant intergroup differences in separated fragment length and location.

Conclusions

Within the limitations of this study, operator experience level appears to have no effect on the lifespan of the WaveOne Primary file in preparation of moderately curved canals. Single teeth with multiple canals can be prepared safely even by a novice operator by using a single file.

Objectives

The study was to compare the shaping ability of Reciproc (VDW) and WaveOne (Dentsply Maillefer) instruments compared with ProTaper, Profile and hand instrument during the preparation of simulated root canals.

Materials and Methods

Five groups (n = 5) were established. Reciproc, WaveOne, ProTaper, Profile and K file (K-flexo file) were used to prepare the resin simulated canals. A series of preoperative and postoperative images were taken by a microscope and superimposed in 2 different layers. The amount of resin removed from both the inner and the outer sides of the canal was measured to the level of 10 mm from the apical tip, with a 1 mm increment.

Results

The mean of resin removal from the inner canal wall was not different from the outer canal wall for Reciproc and WaveOne groups at apical third (1 - 3 mm level). There was no difference in the change of working length and maintenance of canal curvature. NiTi instruments are superior to stainless-steel K file in their shaping ability.

Conclusions

Within the limitation of this present study, Reciproc and WaveOne instruments maintained the original canal curvature in curved canals better than ProTaper and Profile, which tend to transport towards the outer canal wall of the curve in the apical part of the canal.
